Program name: Saguaro Foundation Home & Community Based Services
Description : Community based developmentally disabled persons are able to stay in Saguaro Foundation's group homes on a short term basis in emergency situations or for care giver relief. Respite services are also available directly in the consumer's own home.

Trained and screened staff members provide respite services, which include training and assistance in activities of daily living skills such as bathing, toilet skills, chores, house keeping, personal hygiene, etc.

Independent and semi-independent individuals may receive in-home habilitation services through Saguaro's attendant care and in-home habilitation programs. After assessment of each eligible individual, Saguaro will offer staff persons to assist the individual in obtaining his/her own living arrangements, if needed. Staffing levels are provided based on need, up to 24 hours per day if necessary.

Eligibility Requirements: Individuals must meet Arizona Department of Economic Security/Division of Developmental Disabilities eligibility requirements.
Intake Procedure: Intake is completed by Arizona Department of Economic Security/Division of Developmental Disabilities.
