Description : The Arizona Early Intervention Program provides support to families of children birth to three years of age with disabilities or special developmental needs, helping them to build on their child's strengths, and to enhance and promote the child's growth, development and learning.
Hours of Operation: 8am - 5 pm
Eligibility Requirements: Child must be between the ages of 0 - 36 months. AzEIP eligibility criteria is based upon a 50% delay in one or more areas of functioning (physical, communication, cognition, social/emotional, self-help/adaptive) or have an established condition which has a high likelihood of developmental delay (which includes but is not limited to: chromosomal abnormalities, significant auditory impairment, intraventricular hemorrhage, cerebral palsy, significant visual impairment, metabolic disorders, neural tube defects, periventricular leukomalacia, hydrocephalus, severe attachment disorder, or failure to thrive. Informed clinical opinion can override the 50% designation.

Insurance Information: AzEIP will seek permission to bill a family's insurance company
